They were also cited for a failure to train workers concerning fall hazards and a failure to develop an accident prevention program. The roofing company was also cited for unsafe ladder use and a failure to provide eye protection for workers using nail guns. The company has been cited by the Occupational Health and Safety Administration (OSHA) five times since 2008 for fall violations.
